We are looking for 2 experienced Dietitians to join the team in the position of Program Dietitian. We have a perm full time position available as well as a part time (0.8 FTE) a fixed term contract until Oct 2025.

In this role you will deliver the Type 2 Diabetes and Better Knee Better Me Health Coaching Programs to eligible participants to support weight loss management and Chronic Disease Management.

 

In this work from home role, you will work with a caseload of participants via digital delivery (phone and video platforms). Using your clinical knowledge as well as a solid understanding of health behaviour change methodology, you will empower participants to achieve positive change within their health and wellbeing and assist them better manage their condition and weight loss goals.

 

Key responsibilities in the role include, but are not limited to:

  • Conduct initial health assessments and develop plans to meet participant’s health goals;  
  • Schedule appropriate follow up consultations via telephone or video to deliver a person-centred,  effective and seamless weight loss support program;
  • Provide health and risk assessments such as basic clinical measurements (e.g. blood pressure, height, weight, and BMI etc) and quality of life data and supporting participants to self-report and self-monitor;
  • Generate participant Weight Management plans that are based on program methodology and best practice evidence;
  • Embed health behaviour change principles into program delivery and conduct regular health coaching sessions to support participant to meet weight loss goals and or manage their chronic illness/es;
  • Provide support for evaluations required to show the clinical, operational and financial outcomes of the program;
  • Build and maintain effective relationships with a variety of internal and external stakeholders.

 

Our service operates between the hours of 09:00AM - 5:00PM (AEST) Monday to Friday.

 

About You

You will present with an evident passion for preventative health and a natural ability to build rapport and communicate with a wide range of participants and healthcare professionals.

Working in this highly autonomous position you will have an organised and flexible mindset that allows you to meet the unique needs of your clients.

You are an DAA registered Dietitian with the following experience:

  • Experience in the delivery of health coaching and the delivery of seamless and effective Dietitian support services to people with Type 2 Diabetes and/or Knee Osteoarthritis and other Chronic Illnesses.
  • 5+ years chronic disease management experience as a dietitian
  • Experience in health care and planning specifically relating to weight loss (VLCD) programs or Dietitian based interventions.
  • Experience in remote delivery programs- over phone or video.
  • Solid understanding of health behaviour change methodology and how this can be applied in practice.

 

You will be working from home so will need to have a suitable office area from which to work and reliable internet connection and you can be located anywhere in Australia.
