Job Overview

We invite you to join our dedicated team, where your contributions will help strengthen the vital connection between patients and their primary care providers, ultimately fostering enhanced health and well-being. In this position, you will play a crucial role in guiding patients through their healthcare journeys, addressing their concerns, and improving communication with their medical practices.

Key Responsibilities

  • Collaborate remotely with the staff of patients' medical practices and our Case Management team to manage a specific patient population.
  • Function autonomously while also being an integral member of the team, serving as an extension of the medical practice.
  • Assist patients in achieving improved health outcomes by responding to their inquiries and concerns, linking them with their medical practices, executing effective care plans, and closing care gaps. You’ll also escalate medical issues when necessary and direct patients to the appropriate resources, including specialty care and community-based organizations.
  • Accurately record patients' medical histories and issues in the electronic health record (EHR).
  • Comprehend the referral and prior authorization processes to facilitate patient care.
  • Cultivate and sustain trusting, professional relationships with assigned medical practices and within our organization.
  • Meet designated Key Performance Indicators to promote positive patient health outcomes while minimizing medical expenses.
  • Engage high-risk patients using effective communication methods, including calls, texts, portals, and emails.
  • Appropriately escalate medical concerns to the Health Guide Registered Nurse, Primary Care Physician, or leadership.
  • Occasionally travel to provide support to primary care physician offices.
  • Adhere strictly to all security policies and procedures to protect all Protected Health Information (PHI) and company intellectual properties.

Required Skills

  • Bilingual (Korean/English) - Native or fluent proficiency is essential.
  • Certification as a Medical Assistant or as a certified EMT is mandatory.
  • A genuine passion for patient coordination and a commitment to assisting individuals with complex medical and social needs.
  • Empathy and a willingness to refine skills in motivational interviewing, critical listening, and behavioral change management to enhance patient engagement.
  • Exceptional communication abilities and the capacity to document patient interactions accurately and efficiently using software.
  • Familiarity with medical terminology and the operations of primary care practices.
  • Knowledge of social determinants of health.
  • Proficient in documenting within electronic health records (EHRs) and using care management analytics databases, along with various software programs.
  • Strong ability to manage competing priorities while maintaining a detail-oriented focus.
  • Familiarity with various electronic health records systems such as EPIC, Athena, and Cerner.
  • Excellent written, visual, and verbal communication skills.
  • Ability to track multiple tasks with timely completion.
  • Capability to work independently as part of a patient's overall care team, under the supervision of licensed clinical professionals.
  • Proven success in a remote work environment.

Compensation And Benefits

The base pay range for this role is $24+ per hour, paid bi-weekly. Compensation may vary based on factors including skill set, years of experience, education, location, and certifications.
