Job Summary:


Maintain the integrity of the master patient index and facilitate synchronization of all impacted databases throughout the organization. When duplicate patient information is identified, assure the medical record content is promptly and accurately merged to facilitate patient care. Responsible for oversight and integrity of patient data, as well as day to day clean up and correction activities in the system-wide EHR. Responsible for data quality assessments and coordination of root cause analysis and development of corrective actions. Assists various managers with reporting and some secretarial duties.

Core Responsibilities and Essential Functions:


Maintain patient data systems and related records
* Develop and maintain procedures for selecting the surviving medical record number. Identify & merge duplicate medical record numbers within the system-wide EHR as appropriate and according to established policies & procedures, using Duplicate Patient Report and Mark for Merge Report.
* Develop and maintain procedures for merging medical records to the surviving medical record number. Merge all duplicate medical record numbers and other associated patient information maintaining the accuracy and integrity of electronic and manual records, assuring complete clinical information is only available within the surviving medical record number.
* Assist with Transitions of Care workqueue to assure outside documents are available for patient care in a timely manner.
* Assist with Mark for Merge workqueue.
* Assist with Potential Overlay report to assure demographic changes made in error are corrected accurately and in a timely manner.
* Monitor failed merges to assure quick and accurate resolution of failed merges, and resubmit merge
* Assist with education and daily functions to ensure the accuracy & integrity of our EMPI. Responds to Data Integrity phone calls and general e-mail/inbox questions.
Develop and maintain a procedure for identifying and resolving incidents where an individual is identified to have more than one medical record number.
* Duplicate medical record numbers identified at the point of registration are resolved at the time of registration.
* Duplicate medical record numbers identified outside of the registration process are resolved and other departments are notified of duplicates and surviving numbers to assure synchronization of indices throughout the organization.
* Clinicians and departments treating an inpatient with duplicate medical record number(s) are promptly notified of the duplicate medical record number using Duplicate MRN patient highlight to assure access to all clinical information for patient care.
* End users identifying duplicate medical records needing merging for clinical patient care, using Mark for Merge functionality.
* 24 hour email notification to Home Health and Hospice clinicians prior to merging duplicate medical records containing recent Home Health or Hospice encounters.
* Mark for Merge functionality to assure quick and accurate resolution of duplicate MRNs that have been identified by end users.
Assist Director and Managers with various reports, statistics, and communications.
Performs other duties as assigned
Complies with all Wellstar Health System policies, standards of work, and code of conduct.

Required Minimum Education:


High school diploma High School Graduate Required and
Graduate of a health information technology (HIT) Program Preferred

Required Minimum License(s) and Certification(s):


All certifications are required upon hire unless otherwise stated.

Additional License(s) and Certification(s):

Required Minimum Experience:


Minimum 5 years experience working in a Health Information Management role Required and
Experience working with electronic health records and patient registration systems Preferred

Required Minimum Skills:


Required to type 50 wpm and have working knowledge of Word, Excel, and PowerPoint.
Must have excellent communications skills.
Must be able to communicate effectively with various levels of staff within organization.
Strong analytic and computer skills.
Must be able to determine root causes for data integrity issues.
