Job description

At Counterpart Health, we are transforming healthcare and improving patient care with our innovative primary care tool, Counterpart Assistant. By supporting Primary Care Physicians (PCPs), we are able to deliver improved outcomes to our patients at a lower cost through early diagnosis and longitudinal care management of chronic conditions.

We are seeking a highly organized, results-driven Program Manager to build the operational foundation and manage our most critical programs, starting with complex, large-scale customer implementations. This is a pivotal role for someone who excels at driving clarity and execution in fast-moving environments. You will work cross-functionally to align internal and external stakeholders, build scalable systems, and ensure our programs are executed with discipline and precision. From rolling up your sleeves on project plans to briefing senior leadership, you will be the engine that keeps high-priority initiatives moving forward and delivering impact.

As a Program Manager, you will:

  • Lead high-priority programs: Own and execute large customer implementations and other strategic programs, ensuring operational success from planning to launch and beyond.
  • Build executional rigor: Translate high-level goals into clear plans with work streams, owners, deliverables, and timelines. Keep the team focused on outcomes.
  • Roll up your sleeves: Jump in wherever needed – whether it’s solving tactical blockers, facilitating meetings, putting together high-quality deliverables, chasing down or executing on open items. Maintain altitude and detail without losing either.
  • Drive alignment across stakeholders: Influence without authority. Coordinate with internal teams and external partners/customers to ensure seamless execution, and drive accountability through clarity. Align internal readiness to external dependencies.
  • Keep leaders on-track: Provide transparency, updates, and issue tracking for leadership. Proactively escalate risks and autonomously drive decisions.
  • Translate Ambiguity Into Action: When no playbook exists, build one. When priorities are unclear, force rank them. When coordination breaks down, rebuild the system.

Success in this role looks like:

  • First 90 days: Quickly grasp CPH’s business model, establish key relationships, implement a structured framework for large customer implementations.
  • First 6 months: Successfully drive large customer implementations, develop systems for improving operational efficiency, and identify 1-2 additional high-priority initiatives.
  • First year: Create scalable coordination mechanisms that improve execution across teams, resolve strategic misalignments, and deliver measurable business impact on the operational execution of CPH’s go-to-market strategy.

You should get in touch if:

  • You have had 3-8 years of experience at a consulting firm, in BizOps at a tech company, or in a high-trust internal strategy/Chief of Staff-type role.
  • You understand value-based care and healthcare operations in payor and provider organizations.
  • You think in structured logic, write with precision, and speak in synthesized insights.
  • You’re a system builder who wants to see the impact of your thinking play out in operations.
  • You’ve driven complex, multi-org initiatives to the finish line – not just advised on them.
  • You’re energized by ambiguity, interested in building processes from the ground up, and thrive in a fast-paced environment.
  • You don’t need positional authority to lead. You earn trust and create alignment through clarity, quality, and velocity.
