Responsibilities:

The Community Health Educator, Senior supports the development and execution of the outreach strategy in a territory by participating in events, assisting with SDOH Surveys, providing health education and resources, and documenting outcomes in Jiva for the purposes of impacting health outcomes, health care costs, and member engagement.;

We will be hiring to support the following Regions within Michigan;

  • Region 5/6: Saginaw, Shiawasee, Genesee and surrounding counties (may include travel to Midland, Bay)

The Community Health Educator Senior may be asked to focus in one or all of the below functional areas.;;

  • Community Programs & Events
  • Community Education
  • Member outreach

The Community Health Educator, Sr may work in the field as well as Wellness Centers (brick and mortar and mobile) to support members and member events, programming, and other initiatives.

Additional responsibilities include:

  • Develop health education programs, implement and maintain already existing health education programs, and collaborate with providers to hold screening events for members.
  • Mentor, coach and provide guidance to Community Educators, as assigned by their supervisor.
  • Will be required to provide and pick up supplies for various community events. ;
  • Other duties as assigned by their supervisor.
  • This position will work a 40 hour week and require evening and weekend work.;

Education/Experience:

  • Bachelor's Degree required;
  • Ability to travel (75%) extensively within assigned communities.; Valid driver’s license and access to an adequately; insured vehicle (per state requirements) required
  • Additional licenses, if/as required by the state
  • Three to five years;health education, marketing or public health experience.
  • One to Three years of Managed Care experience.;

Other Skills:

Knowledge of healthcare industry and managed care preferred.
