Overview:

Sentara Health is seeking a dedicated Integrated Nurse Case Manager/RN/OBGYN - Lower Central region of Virginia!

This full-time, work-from-home position requires travel to conduct face-to-face home visits in the member’s home within the lower Central region of Virginia.

This includes but is not limited to the following areas: Petersburg, Colonial Heights, Hopewell, Emporia, Dinwiddie, Prince George, Amelia, Nottoway (Blackstone), Brunswick (Lawrenceville), Lunenburg (Kenbridge, Victoria), Mecklenburg (South Hill), Greensville, and Sussex County.

Applicants must reside in or near one of these locations to be considered for the role.

Education
  • BSN (Bachelor of Science in Nursing)  preferred
Certification/Licensure
  • Registered Nurse License (RN) - Nursing License - Compact/Multi-State License require

Experience

  • Nursing experience, with a focus on case management, care coordination, or patient advocacy, Medical/Clinical cases for pregnant and postpartum women.
  • Maternity experience (L&D, Postpartum, Mother/Baby)  is preferred.Maternity experience (L&D, Postpartum, Mother/Baby)  is preferred.
  • Strong knowledge of physical, psychological, socio-cultural, and cognitive patient needs.
  • Excellent communication skills, both oral and written, as well as strong problem-solving and analytical skills.
  • Ability to work independently and collaboratively in a remote, fast-paced environment.

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay rate for Full Time employment is:$66,830.40-$111,384.00Additional compensation may be available for this role such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
