Build awareness about the NYHER (New York Health Equity Reform) program and Medically Tailored Meals (MTM) from Mom’s Meals (MM) throughout New York communities and the importance of nutrition and its impacts on Medicaid beneficiaries. Possess a passion for ensuring that beneficiaries are aware of and able to access available services that can improve their lives. Create an organized network of contacts and target organizations within internal systems. Engage and participate in Social Care Network Meetings and Webinars. Attend and grow network of community events. The successful candidate will drive an increase in MTM referrals from a variety of organizations throughout NY. Build strong relationships with Community Based Organizations, Enhanced Care Management (ECM) Providers, Clinics, Hospital and Health systems, etc.
This position will be based in the Albany, Poughkeepsie, or New York City area so candidate must reside in that area. It will also be eligible for a company car as it will travel 50%.
Salary range: $60,000-80,312
