As an RN on the Home Care Team, you will deliver the best possible support to patients in the comfort of their own homes. In this role, you will provide assessments, complex treatments and education to help avoid readmissions to the acute care setting and ensure comfort. This position offers a flexible schedule, allowing you to maintain a healthy work-life balance, all while changing the lives of your patients and their loved ones every day. 

A caregiver in this position works varying shifts and required to work a minimum of 48 hours every 6 weeks (includes one weekend shift, if needed) and 1 holiday a year. You will be required to complete 8 weeks of full-time orientation (Monday - Friday, 8a- 5p).

A caregiver who excels in this role will:  

  • Provide direct nursing care in accordance with established healthcare organization policies, procedures, and protocols. 
  • Implement and monitor patient care plans.  
  • Provide supervision to LPNs, Home Care Aides and families/caregivers involved in the care of the patient. 
  • Perform accurate and complete admission assessments and reassessments within a specified time frame. 
  • Assess patient for safety risks, abuse and neglect. 
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care. 
  • Assess and document patients' pain using appropriate pain scale and manage patients' pain then. 
  • Prioritize and document patient problems based on assessed needs and problems in collaboration with patients, family and healthcare team. 
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions. 
  • Escalate change in patient condition to patient’s physician as appropriate. 

Minimum qualifications for the ideal future caregiver include:  

  • Graduate from an accredited school of Professional Nursing 
  • Current state licensure as a Registered Nurse (RN) 
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA) 
  • One year of clinical experience as an RN in an acute medical/surgical setting 
  • Six months of experience as a Homecare Nurse 
  • Valid Driver’s License (VDL) in Ohio and proof of automobile insurance with $100,000/$300,000 coverage 

Preferred qualifications for the ideal future caregiver include: 

  • Bachelor’s of Science in Nursing (BSN) 
  • Infusion therapy experience

Physical Requirements:

  • Requires full range of body motion including handling and lifting packages, manual and finger dexterity, and eye-hand coordination.
  • Requires standing and walking for extensive periods of time.
  • Heavy work – Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects.  Physical Demand requirements are in excess of those for Medium Work.

Personal Protective Equipment:

  • Follows standard precautions using personal protective equipment as required.
