Job Description:
Responsible for leading projects that improve workflows and/or address issues in the daily operations of the Population Health division. Represents the division with both internal and external stakeholders in developing programs, resolving issues and proactively researching workflows to improve operations and service delivery. Responsibilities include implementation of programs; acting as an expert resource; mentoring and training staff; taking a leadership role in activities that contribute to meeting the mission, goals and service expectations of Population Health. Also responsible for providing information and developing protocols to assist with meeting the requirements of accrediting agencies. Provides input to the leadership team regarding protocols, staffing, policies, procedures, systems and operations. This position requires in-depth system information and knowledge.

Education Qualifications:

Key Responsibilities:

  • Develop detailed project plans outlining tasks, timelines, and resources
  • Schedule regular meetings with the project team to review progress and address any issues
  • Allocate resources effectively to ensure project tasks are completed on time
  • Facilitate collaboration and resolve any conflicts or issues within the team
  • Monitor risks throughout the project lifecycle and adjust plans as needed
  • Track project expenses and ensure the project stays within budget
  • Maintain regular communication with stakeholders to provide updates and gather feedback
  • Prepare and present project status reports to stakeholders
  • Maintain comprehensive project documentation, including plans, reports, and meeting minutes
  • Ensure all project information is up-to-date and accessible to the team
  • Implement corrective actions to keep the project on track
  • Track project progress against milestones and deadlines
  • Evaluate project outcomes and identify areas for improvement
  • Implement lessons learned in future projects to enhance efficiency and effectiveness

Education Requirements:

  • Bachelor’s degree in healthcare or related field

Required Qualifications:

  • At least 3 years of project management experience

Preferred Qualifications:

  • Project Management Professional Certification or other project management experience including SCRUM or Agile
  • Experience leading and implementing organization change, setting performance measurements, establishing plans and monitoring processes, program development and process
