Job Brief:
Pfizer Specialty Care Patient Solutions is seeking a compassionate, strategic, and organized Patient Navigator (PN) to join our U.S. Patient Affairs Team. The Patient Navigator will serve as a guide for patients through their healthcare journey by providing resources to help ensure they understand the treatment landscape and available resources. The PN will also be fully and appropriately integrated into Pfizer’s current cross-functional ecosystem ensuring that all patients can receive the appropriate support resources they want and need. The PN will compliantly triage patients as appropriate to applicable reimbursement support resources.
The PN role is a field-based community-facing colleague for individuals living with hemophilia and their caregivers. The primary function of the PN is to compliantly educate patients on the disease landscape and other patient support programs that may be available through Pfizer. The PN will be the single point of contact with the patient and will function as a liaison to either provide patients with internal or external resources. The PN role reports directly to the Rare Disease Patient Engagement Senior Director.
The objectives of the Patient Navigator (PN) role are:
Patient Education and Support: The PN longitudinally educates patients and caregivers about hemophilia-related disease state awareness, treatment option modalities, local and regional support offerings, health and wellness resources within a patient’s community, as well as appropriate triage to available resources at Pfizer.
Patient Navigation: The PN assists patients in navigating their overall care and local support systems by connecting them to appropriate local, regional, and Pfizer resources. The PN does not assist with reimbursement support for patients prescribed Pfizer hemophilia therapies.
Collaboration: The PN closely and compliantly coordinates with the Community Engagement Lead (CEL) and other cross-functional partners to ensure that the integrated patient offerings align with intended support and educational objectives in the context of local market dynamics and hemophilia longitudinal patient journeys
Why Patients Need You:
Individuals living with hemophilia and their caregivers may face various challenges in diagnosis, understanding treatments options, obtaining, and maintaining treatments.
Proper disease education and awareness can equip patients to have the appropriate discussions with their healthcare providers.
Particularly in hemophilia with many new emerging therapies (e.g., Gene Therapy, anti-TFPI class), patients need to understand the various options from Pfizer’s portfolio that they can then discuss with their healthcare providers to determine what is best for them
The PN is the central point of contact for patients and caregivers and focused on disease state awareness, education, support resources, and resources along the disease and treatment journey and connects patients to other resources inside and outside of Pfizer as well as other local health and wellness resources in their local community.
What You Will Achieve:
Provide education and guidance to resources for patients to address their questions and need for information
Assist patients in navigating their overall care – via educating and connecting patients to appropriate resources
Share educational resources on emerging treatments, shared decision making and treatment journey
Closely coordinates with the Community Engagement Lead (CEL) lead, attending PAG events within geography to present RC approved education on emerging therapies and treatment options
Through use of motivational interviewing, identify specific patient needs and goals to drive tailored and appropriate engagement for patients / caregivers
For opted-in patients, provides in-person and virtual 1:1 engagement through a defined communication strategy and purposeful use of a curated menu of print and digital resources based on the patient/caregiver’s individual journey:
Build trust and provide ongoing support resources for the patient to take the next best action
Provide educational materials / resources about hemophilia, disease or treatments
Connect patients / caregivers with local health & wellness resources through referrals to local organizations
Appropriately provide patients with educational support throughout the entire treatment journey as appropriate
How You Will Have Impact:
The PN will be located within a specific geography / region that assists patients in navigating their overall care – via educating and connecting patients to appropriate healthcare resources
Understands and strictly complies with all relevant regulations and policies. All activities are executed in close collaboration with legal and compliance to ensure that they are implemented in a thoughtful and compliant manner that meets Pfizer’s high standards of conduct
Deliver overview of Hemophilia Patient Navigator role and Pfizer patient support resources to patients and caregivers at various Pfizer-sponsored events (e.g., in-person/virtual booth exhibits) at local, regional, and national levels
Partners with Pfizer Community Engagement Lead to meet the needs of patients and caregivers through providing educational content and resources to local support groups
Engage with opted-in patients / caregivers in a mutually agreed upon manner (e.g., phone calls, emails, texts, in-person, etc.)
Compliantly collaborate with cross-functional partners to ensure that the integrated patient support resources align with intended objectives in the context of local market dynamics
Provide aggregated local insights to inform Rare Disease Team on the development of educational materials for patient engagement and patient support resources that fill needs within the patient community
Compliantly schedule and organize travel in a timely, thorough manner and order all required materials for local, regional, and national events
Outcome:
The PN will work toward defined measurable goals of patient engagement shared across cross-functional teams, as appropriate, inclusive of Pfizer CEL, ARM, PCM, and potentially external stakeholders such as HUB providers)
Held accountable to PN established goals.
Basic Qualifications:
B.S. or B.A. Degree is required
Minimum 7-10 years pharmaceutical industry or related experience - preferably in Rare Disease
Experience demonstrating exceptional communication, listening, and platform trained speakers
Demonstrated ability to work in a highly regulated environment and adhere to strict company compliance guidelines and procedures
Proven track record of collaborating broadly with a diverse range of customers, clients, and stakeholders
Experience in responsible business practices and compassionate communication both written and spoken
Understands and navigates Pfizer and industry compliance guidelines aligned to working with MO&A (Medical), and Access & Reimbursement colleagues
Ability to deliver meaningful & concise presentations with integrity and empathy
Strong attention to detail, organizational and business skills
Ability to travel minimum of 50%; scope of events is primarily on weekends and evenings
Preferred Knowledge & Skill Sets Desired:
Clinical background is preferred (RN, APRN, NP, ARNP, ACSW, LCSW, MPH)
Extensive knowledge of the Rare Disease community
Remote / Field based, preferred to live in specified geography
Certain areas require bilingual proficiency in speaking and reading
Proficiency with CRM tools (e.g., POLARIS CRM)
