About The Role
As a Senior Support Coordinator, you will provide high-level, person-centred support to individuals with complex needs. You will assist participants in navigating the NDIS, connecting with services, and building capacity to live more independently. This role also involves mentoring junior staff, driving service excellence, and contributing to innovative care models.
Key Responsibilities
Advanced Support Coordination:
- Guide NDIS participants in accessing services that align with their goals.
- Assist with resolving service delivery challenges and complex cases.
- Develop and implement tailored support plans, ensuring they evolve with participant needs.
- Build and maintain relationships with stakeholders, including participants, families, and service providers.
Mentorship & Leadership:
- Provide coaching and guidance to junior support coordinators.
- Foster a collaborative team environment focused on continuous improvement.
- Keep the team updated on NDIS policy changes and best practices.
Operational Excellence & Compliance:
- Maintain accurate documentation and service agreements.
- Ensure adherence to NDIS guidelines, funding requirements, and compliance standards.
- Contribute to the development of policies and procedures that enhance service delivery.
About You
Experience: Minimum 3-5 years in Support Coordination, case management, or a similar role within the NDIS sector.
Knowledge: Strong understanding of NDIS legislation, plans, and funding structures.
Skills: Excellent problem-solving, communication, and relationship-building abilities.
Tech Proficiency: Familiarity with NDIS systems, CRM platforms, Google Suite, and Microsoft Office.
Qualifications: Relevant degree in Social Work, Psychology, Disability Studies, or a related field (or equivalent experience).
