Director, Transformation & Innovation Services, Population Health

The Role

The Director, Population Health will be part of the EHR Innovation and Integration team within Transformation & Innovation Services. This position will be responsible for successfully managing population health and value-based care strategies and initiatives for enterprise clients and be a visible project leader supporting people, processes, and technology. This role is instrumental in improving client daily operations while simultaneously looking at the larger strategy for continuous alignment and harmonization of common goals and business outcomes and develop strategies to optimize workflows and implement solutions for enterprise population health areas. In addition, the Director, Population Health enables and collaborates with the sales team to position Nordic’s population health offerings to our broader customer base.

This position works under the direction of the Director, Transformation & Innovation Services, EHR Innovation and Integration to address assigned client priorities while actively seeking opportunities to refine client deliverables, build knowledge in new domains, and improve Nordic’s business processes.

Responsibilities

  • Manage the rhythm of the account – play a key part in the operational activities of the Enterprise Account senior leadership team, enabling the team to be more productive and successful.
  • Convey decisions and facilitate actions in a manner that reflects the Enterprise Account Leader’s direction, relaxing the dependency on SVP’s direct involvement or feedback to enable progress on critical initiatives.
  • Maintain a tracker of executive team initiatives (living document) to keep a pulse on progress.  Provide regular read-outs to communicate status, accomplishments, and red flags.
  • Participate in quarterly planning sessions and annual performance planning cycles, including delivery, financial, or client relations improvement initiatives for Enterprise Accounts
  • Meet regularly with the account senior leadership team members (and others as necessary) to facilitate progress on open initiatives and help address roadblocks.
  • Manage population health and value-based care activities to support the organization and company’s goals, objectives, and outcomes.
  • Manage new client-driven population health or value-based care programs and projects to ensure timely completion of projects and communicate changes to current operations and delivery.
  • Develop strategic roadmap and manage prioritization with client executive teams.
  • Evaluate and make recommendations to clients on future population health platform strategy.
  • Identify, evaluate, recommend, and drive the analysis and resolution of gaps, issues, and risks.
  • Analyze current technical and operational processes to recommend further efficiencies and process development and partner with respective owners.
  • Provide direction and guidance to team members (internal and external).
  • Assist in continuous refinement of Account Operations processes and incorporate lessons learned including internal and external feedback.
  • Develop presentations and summary reports for various stakeholders and facilitate discussions to raise and resolve issues, risks, and escalations.
  • Advise client executives on key business decisions and major issues based on program and project activities.
  • Act as a population health or value-based care subject matter expert in a customer-facing role.
  • Collaborate with internal Nordic leadership and EHR architects on solution design and delivery.
  • Provide pre-sales solution generation that succinctly describes how a solution meets the client’s communicated needs and engenders client support for the proposed solution.
  • Elicit customer requirements, visioning solutions, and documenting technical and business requirements pertaining to population health initiatives.
  • Lead solution development processes through continuous and iterative solution planning, scoping, and documentation, team mentorship, development review, and client discussions.
  • Present strategic guidance for enhancement of client products or go to market execution.
  • Improve process and internal support documents and assisting in the improvement of workflows and processes and managing multiple projects simultaneously.
  • Coordinate and forecast work for self and others working on owned solutions.
  • Participate in continuous learning opportunities.
  • Work to uphold and strengthen Nordic’s corporate culture.

Requirements

  • Master of Business Administration or master’s degree in related healthcare or technology field. 
  • Ten (10) plus years of Healthcare IT experience.
  • Active Epic Healthy Planet and/or Compass Rose certification.
  • Experience managing people, programs, and projects within healthcare and IT environments.
  • Experience working as a consultant, lead analyst, product manager or software engineer required.
  • Proven experience creating execution plans, risks, issues, actions items, and escalations.
  • Proven experience planning and managing continuous improvement around people, processes, and technology and understanding potential impact (cause and effect).
  • Strong analytical, problem-solving, and attention to detail skills.
  • Ability to collaborate across internal and external teams.
  • Present data, findings, and status to key stakeholders and communicate to and influence leadership based on recommendations.
  • Proven experience developing and presenting project status and executive summary.
  • Experience managing Healthcare transitions and transformations a plus.
  • ITIL and PMP certification a plus.
  • Exceptional customer management and service skills
  • Excellent communication skills, written and verbal
  • Demonstrated ability to seek out, analyze, and solve novel problems
  • Ability to organize with exceptional attention to detail
  • Able to work independently and within a team environment
  • Must demonstrate and embody Nordic’s maxims

Additional Details

  • 100% remote position
  • Travel up to 10% of the time including to home office in Madison, WI
