Senior Care Coordinator 

Xyla Community Services (part of Acacium Group)

Remote

Permanent 

Monday - Friday between 8am - 6pm aswell as occasional weekends.

£27,000 per annum

We are looking for a Senior Care Coordinator to join our team, where you will play a vital role in our company's success. In this role, you will closely collaborate with hospital discharge teams and care homes to source the best care packages for our clients.

This position offers a unique opportunity to work towards quick and safe discharge plans, supporting families during challenging times.

Every day you will…

  • Sharing updates and acting as the link between services and service users.
  • Provide support to the Care Liaison Advisors within your team. Supporting the Scheme Manager in managing day to day activity.
  • Interact with families in a caring and empathetic manner.
  • Act as the liaison between discharge teams, local authorities, clients, and care providers, ensuring timely and high-quality service delivery within the established timeframe.
  • Ensure prompt and precise discharge plans for patients and their families
