The Job
The Care Management Coordinator will support Integrated Care Case Managers in the day-to-day clinical operations to improve member outcomes and timely care coordination in partnership with and under guidance of the Integrated Care Delivery Care Team according to Care Management protocol, policy, and procedure.
You Will
Manage daily Inpatient contracted/non-contracted hospital census and Skilled Nursing census; confirm admissions/discharges and reconcile census.
Close cases for inpatient/skilled nursing discharges and send out appropriate documents to patients and Primary Care Physicians.
Support coordination of timely delivery of last day cover letters to skilled nursing facilities according to Care Team protocol.
Support coordination of discharge needs from inpatient acute settings and skilled nursing facilities as appropriate for each member. Discharge needs may include: durable medical equipment, home health services, specialist referrals, skilled nursing placement, transportation, etc. according to Care Team protocol.
Review tasks assigned by Integrated Care Nurse Practitioners and coordinate member needs accordingly such as: home health request, wound care supplies, Foley supplies, colostomy supplies and outpatient services such as blood transfusion or interventional radiology services.
Support coordination of direct admissions to skilled nursing facilities from assisting living facilities or board care setting according to Care Team protocol.
Support coordination of ER transfer versus direct admissions to inpatient setting from assisted living facilities or board care setting according to Care Team protocol.
Track patients referred to hospice care, assign task in EHR and follow-up each month to verify hospice status.
Manage faxes received in Medhok system.
Promote the mission, vision, and values of SCAN Health Plan and serve as an ambassador when interfacing with external stakeholders.
Actively support the achievement of SCAN’s Vision and Goals.
This position is a patient-facing role.
Employees may be eligible for differential pay.

Your Qualifications
- High School Diploma or GED
- Bachelor’s degree, preferred
- 2+ years of experience as an care coordinator in care management department
- Ability to communicate effectively verbally and in writing
- Highly organized with attention to detail
- Resourceful problem solver
- Strong interpersonal skills, hospitality, and change agent mindset
- Adaptable and open to change
