Working Title: Population Health Specialist

FLSA Status: Non-Exempt

Posting Salary Range: $33,820 - $43,497

Office Location: Remote within North Carolina.

What We’re Looking For

Trillium Health Resources is seeking a dedicated and passionate Population Health Specialist to join our team and make a tangible impact on the lives of individuals within our community. The Population Health Specialist position is a key component of our expanding population health strategies. This role will primarily function telephonically and virtually, allowing you to connect with members from the comfort of your workspace. You will play a crucial part in engaging members in our Tailored Care Management program, ensuring they receive the personalized support they need to thrive. On a typical day, you might:

  • Outreach to support care gap closures and other organizational identified engagement initiatives.
  • Engage members to support survey collection of population health program participation
  • Offer member education on topics such as health and wellness concepts and how to access community resources.
  • Assist with the development and coordination of projects.
  • Perform supportive outreach, follow-up, and engagement with members.
  • Ensure the proper storage and transmission of sensitive health records.
  • Provide any additional support services/referrals in addressing unmet health-related resource needs, such as housing, transportation, nutritional food sites, and other services as needed.
  • Document activities in the Trillium Connections care management platform. Documentation should be timely, accurate, complete, and reflective of the member’s needs.
  • Review member information within the platform, update information as needed, and/or inform the appropriate staff when inaccuracies are found.

Qualifications: 

Required Education: High School Diploma/GED

 

Required Experience:

 

  1. Have one (1) year of paid experience in the Health and Human Services field, including experience working with any combination of the BH/SU/IDD population with at least one year of paid experience working directly with people experiencing serious and persistent mental illness, severe or prolonged substance use, intellectual or developmental disabilities.  Duties such as experience teaching BH/SU/IDD population students, or working with them in institutional settings, performing general outreach with individuals, coordinating services/appointments, engaging in health promotion activities, linking members to community resources, and supporting a care team would be qualifying experience; or

 

  1. Have one (1) year lived experience with an intellectual/developmental disability (I/DD) or traumatic brain injury (TBI) with demonstrated knowledge/direct personal experience pertaining to the North Carolina Medicaid system; or

 

  1. Have one (1) year of lived experience with a behavioral health condition with certification as a Peer Support Specialist; or

 

  1. Have one (1) year of being a parent or guardian of an individual with an I/DD, TBI, or behavioral health condition with direct experience providing care for that individual and navigating the Medicaid system on behalf of that individual note that a parent/guardian cannot serve as a Health Specialist for their family member).

Preferred Experience:
 
Two years’ experience is preferred. Prefer lived experience, experience working with people who have serious and persistent mental health conditions, or experience with community-based organizations centered around housing, food insecurity/healthy eating, transportation, education & employment, interpersonal violence/toxic stress, and/or social integration and inclusion.

 

Required License/Certification: 

Certified Peer Support Specialist if used for qualifying as noted above.

Must have a valid driver’s license.

 

Preferred License/Certification:
Prefer Certified Peer Support Specialist, CNA, or Community Health Worker (CHW) certifications.


Location:
 Remote within North Carolina. Must reside in the region assigned.

Must reside in North Carolina to be considered for remote status.
