Job Description

Graham Healthcare Group is seeking a Home Health Operations Manager who has excellent problem-solving abilities and plays a key role in ensuring clear and documented external resource management that supports the home health operations. The role is responsible to manage, track, and report on day-to-day activities of business processes to ensure timely and accurate completion of designated work to external partner relationships under the direction of the SVP Clinical Integration and Performance.

Home Health Operations Manager Responsibilities:

  • Implements scalable processes using an integrated external partner to enhance clinical operations

  • Analyzes home health operational volumes and coordinates with external partner for appropriate resource allocation 

  • Coordinate the planning and implementation of global partner integration into home health operations, including assistance in preparing and maintaining checklist for pilot projects

  • Facilitates setup for external partner provision or deprovision of system and tool access to assure timely completion of job tasks within HCHB, SHP, Medbridge, and other required Graham associated tools. 

  • Establishes the playbook of job aids and standard operating procedures by collaborating with Subject Matter Experts (SMEs) when needed to promote clear working requirements that meet Graham’s operational needs

  • Manages day to day operations by serving as a liaison between external partner and home health sponsors

  • Conducts training for new or changed processes

  • Using approved training references, leads to ensure external partners have a clear understanding of expected performance

  • Uploads and manages reference library of resources timely to Graham hosted Share-point site for partner

  • Proactively communicates to both internal and external key stakeholders that promote continuous alignment of expectations

Home Health Operations Manager Requirements:

  • Relevant home health or clinical operational project management experience

  • Must possess competency and time driven methods in managing multiple tasks, prioritizing, and finishing complex projects

  • Must be willing to flex schedule to interact with global partners

  • Impactful written and verbal communication skills; ability to set clear project direction, develop presentations and key deliverables, escalate risks, and influence key stakeholders inclusive of internal leadership

Home Health Operations Manager Experience:

  • Bachelor’s degree required in Healthcare Administration, Business, or Clinical related field

  • Minimum of 5 years of experience required in home health or clinical setting

  • Project Management certification preferred

  • Relevant experience with focus on process performance improvement initiatives and change management highly recommended
