The FACT Case Manager provides outreach and referrals to parents and families challenged with the effects of substance abuse. The Case Manager links the program participant to appropriate substance abuse intervention including detoxification, rehabilitation, and intensive outpatient treatment. The case manager provides coordination of services between participants, family, and multiple providers and educates and counsels participants and families on issues related to substance abuse, including achieving and maintaining sobriety and relapse prevention.
