This is a remote position.

OpenLoop is seeking Primary Care Physicians specializing in Chronic Care Management to join our virtual team. We provide care ranging from comprehensive preventive care to managing complex chronic conditions, but most importantly support patients and families with compassion and expertise!

About the Position

This is an incredible work opportunity in a fast-paced, innovative company that puts the patient first. We hire clinical care professionals who deliver medical care using telemedicine modalities including video, phone, connected devices, and asynchronous messages (chat/email).

Physicians are members of an interdisciplinary care team working as staff clinicians with OpenLoop. They work independently and are responsible for diagnostic and therapeutic decision-making within the industry and locally established standards of care, clinical practice guidelines, and organization policies and procedures. The scope of medical decision-making focuses on common conditions that would present within an episodic, acute, or primary care medical practice. Responsibilities

  • Obtains histories, conducts virtual exams, documents findings, evaluates and diagnoses medical and behavioral health conditions, recommends treatment plans, constructs care plans, enlists support from other team members and promotes peace of mind by investing in patient confidence

  • Delivers patient care in high-priority care targeting chronic illness, preventative care, aging, and hospice care 

  • Develop, review, and sign off on care management plans that are tailored to individual patient needs, ensuring alignment with best practices and clinical guidelines.

  • Evaluate and approve treatment plans, including medication regimens, lifestyle modifications, and therapeutic interventions, based on patient progress and feedback.

  • Provide guidance on advanced care planning, including durable medical directives and end-of-life preferences.

  • Sign off on care management plans and treatments developed by the multidisciplinary team to ensure clinical accuracy and compliance.

  • Educate patients on self-care strategies and proactive health behaviors to improve long-term outcomes.

  • Delivers patient care that balances the best that medical science has to offer, patient preferences and values, and invests in patients' future confidence and skill



Requirements
  • Board-certified physician (MD or DO) from a US-accredited medical school

  • Board-certified in Family Medicine OR  Internal Medicine

  • Must possess active, non-restrictive medical licensure in multiple states: 

  • MINIMUM of 5 Active State Licenses from the Following:

    • TOP PRIORITY: CA, FL, IL, NJ, NY, PA, and TX

  • A minimum of TWO years experience as a practicing physician is required

  • REQUIRED EXPERIENCE: chronic illness care, preventative care, aging, and or hospice care

  • Must have no history of malpractice

  • Excellent communication and interpersonal skills

  • Outstanding clinical expertise, evidence-based practice

  • Competent in providing patient education at every visit

  • Strong verbal and written communication skills

  • Above-average comfort with technology and assisting patients with troubleshooting technical difficulties that may occur in the home environment

  • Timely and professional

  • Telemedicine or virtual care experience a plus

  • Fluency in Spanish (both written and spoken) is a plus
