Job Summary:
Medical Case Managers assist individuals with chronic health conditions including HIV, to access and engage in medical care and treatment, behavioral health services, insurance, and other community services which support the individual’s wellbeing and reduce barriers to quality medical care. This is a full-time, non-exempt, union, regular position.

Engages and builds rapport with primarily medium to high acuity clients, completes assessments, co-creates service plans, facilitates the acquisition of all appropriate services, and monitors individual service plan progression.
