The Position: 

Job Title:  Outreach Worker, Prenatal Outreach Support Team (POST)

Job Type: Permanent, Full-time 

Work Arrangement: Remote

 

Position Overview: 

The Prenatal Outreach Support Team (POST) is a collaborative initiative between Calgary Police Service (CPS), CUPS and Kindred, to identify and provide specialized intervention and referrals for vulnerable pregnant women in high-risk situations. The POST Outreach Worker position is designed to enhance and assist in providing support and outreach to marginalized people who are pregnant or parenting newborns with cultural awareness and humility. This position reports to the Supervisor, Prenatal Outreach Support Team (Community Based Services).   

  

This position requires a valid driver’s license, clean driver’s abstract and use of a vehicle with appropriate insurance. There is an expectation of flexibility with hours to meet client needs; some evening work required.   

 

Key Accountabilities (job functions include but are not limited to):  


Client Serving (65%)  

  • Provide direct support and engagement with clients; 
  • Maintain a client caseload according to program requirements. 
  • Establish positive relationships with clients that empower them to make decisions in their present situations; 
  • Complete risk assessments for clients; 
  • Use a harm reduction approach to intervene with clients; 
  • Utilize a variety of outreach methods to reach isolated and/or disengaged clients; 
  • Assess client barriers to services and facilitate access as required; 
  • Provide urgent supports to clients based on their need; 
  • Maintain regular contact with clients in the community; 
  • Provide information, education and support to increase both awareness of risk factors associated with pregnancy and a positive outcome for both mother and newborn; 
  • Provide POST assistance and facilitate transportation to access appropriate services; 
  • Engage, collaborate and build trusting relationships with community service providers in and around Calgary to enhance the continuum of services for the clients; 
  • Participate in POST meetings, documentation, education and evaluation activities; 
  • Facilitate culturally sensitive practices with clients; 
  • Work collaboratively with community partners and other program staff; 
  • Participate in staff meetings in a collaborative manner providing linkages between other teams. 

Administrative (25%)  

  • Administrative tasks such as data collection, database input and casework documentation; 
  • Participate in individual and group supervision, team/agency/division meetings, live observations and professional development opportunities; 
  • Consultation with supervisor, colleagues, natural supports and other professionals; 
  • With support of supervisor, coordinate use of holiday time to best suit the needs of the program; 

Agency and Collaboration (10%)  

  • Positively represent Kindred and the Community Programs division with sector partners, donors, volunteers and other stakeholders; 
  • Work collaboratively with our Calgary Board of Education and Alberta Health Services partners as well as other program staff; 
  • Participate in committees and events that support the agency and the division; 
  • Support division needs as they arise. 


Qualifications:


Education and Experience   

  • Bachelor's degree in social work or related Human Services field or a diploma with experience aligned with position; 
  • 3-5 years working with hard-to-reach youth; 
  • Eligible and in good standing with professional governing body if applicable; 
  • Takes a posture of Cultural humility in working with diverse populations; experience with Indigenous and/or Immigrant families is an asset; 
  • Experience with quality assurance, data management, program outcome measurements and evaluation, and co-creation of reports. 

Knowledge, Skills and Abilities  

  • Utilized a Case Management approach to support client needs; 
  • Familiarity with using computerized tools (e.g., zoom, MS teams, databases) for communication and case management is expected; 
  • Crisis management; 
  • Group planning and facilitation skills; 
  • Knowledge of child development and positive parenting practices; 
  • Ability to cross train to another divisional role to provide back up when needed; 
  • Knowledge of adolescent development, domestic violence, substance abuse and mental health; 
  • Knowledge of Trauma-informed practice, Systems theory and working through a family lens; 
  • Knowledge of community resources, women’s health issues including experience with pre and postnatal care and working with clients struggling with addictions, poverty, and homelessness;
  • Computer literacy that will assist in using the HER and Netcare.
