Practice Plus Group are looking for clinicians for the Unscheduled Care Coordination hub (UCCH) in Suffolk and North East Essex.

The UCCH vision is to ensure that ‘People receive the right care, in the right place, first time, every time’ as outlined in the Government paper Next Steps to Put the People at the Heart of Care

To be successful in this role, you will need to have good knowledge of local urgent care, primary care and secondary care pathways and infrastructure and the ability to work collaboratively with SNEE system partners to help deliver better quality of care for patients.

This is the perfect opportunity to use and develop your skills and expertise to provide quality, evidence-based health care to meet the immediate needs of the Suffolk and North East Essex patient population.

The service is operational 7 days a week, 0800hrs-2000hrs with opportunities for full or part-time work (either salaried or bank).
