Job Title: Registered Nurse (RN) - Home Care

Job Summary: We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Care team. As a Home Care RN, you will provide in-home health care services to patients, helping them manage chronic conditions, recover from surgeries, and maintain overall well-being in the comfort of their homes. This role is ideal for a caring professional who values patient-centered care and has a passion for working with patients in a home environment.
Key Responsibilities:
Assess patients' health needs, create personalized care plans, and adjust as necessary.
Administer medications, perform wound care, and provide other necessary treatments.
Educate patients and families on health care needs, safety protocols, and health management.
Monitor and document patient progress and report changes to the healthcare team.
Coordinate with doctors, therapists, and other care professionals to ensure holistic care.
Provide emotional support and compassionate care to patients and families.
Qualifications:
Active RN license in the State of Maryland
Minimum of 1-2 years of experience in a clinical setting (home care experience preferred).
Excellent communication and organizational skills.
Ability to work independently and manage time effectively.
Reliable transportation for patient visits and a valid driver’s license.
