Are you a dedicated and compassionate adult nurse looking to make a difference in the community?  We are seeking a qualified Community Care Registered Mental Health Nurse to join our dynamic team. Our mission is to provide flexible, person-centred care and support to individuals with complex needs, allowing them to be cared for in the comfort of their own homes.

Job Title:           Community Care Registered Mental Health Nurse

Reporting to:    Community Care Nurse Manager

Location:          Remote/Hybrid - working between home and community visits, ideally located around Yeovil, Taunton, Glastonbury or                    Bridgwater areas (some travel to a wider geographical area required e.g. Truro) 

Hours:              Part-time – 3 days per week / 22.5 hours per week 

Salary:              Pro-rata salary for 22.5 hours (3 days per week) approximately £23,100 - £25,500

                         (Up to £42,500 per annum (Full-time equivalent)) 

Other Benefits:  Car Allowance (Pro rata per month, including mileage, travel would only be expected in    work hours)

                           Up to £10,000 pa bonus (pro rata)

Who are you?

 

You will be a Qualified Registered Mental Health Nurse (RMN) and will be dedicated and passionate about making a difference in your community. You will want to be part of a national team of nurses dedicated to providing expert care in the community allowing care and support to individuals with complex needs, enabling them to be cared for in the comfort of their own home. 

 

What will you do?

  • Be physically visible and accessible to service users, families and carers working together to ensure efficient, effective and caring services;
  • Devise, implement and evaluate care plans, risk assessments and associated care documentation, whilst maintaining a person /care centred approach;
  • Work alongside and support the Registered Manager to ensure all referrals are reviewed and assessed, as well as debriefing service users and carers following critical incidents and difficult situations;
  • Maintain a safe service as a trusted voice and advisor to ensure full competency sign off;
  • Support, monitor and be accountable for delegated clinical tasks to Community Care Leads and Healthcare Workers;
  • Work closely with the Community Care Nurse Manager to support the continual improvement of Community Care;
  • Provide clinical leadership, support, supervision and direction to the Community Care Leads and Healthcare Workers to carry out enhanced clinical care duties and risk management; maintaining their clinical training and competencies;
  • Proactively manage and communicate to ensure safe, responsive and effective care is provided to reduce risks;
  • Be solution driven and partake in crisis management of packages of care;
  • Undertake clinical investigations where delegated; and
  • Provide support and guidance out of hours via on-call.

 

The skills that make you different :

  • Qualified Nurse with current Registered Mental Health Nurse (RMN) registration
  • Experience within Mental health settings including but not limited to community, crisis intervention, acute, rehabilitation, forensics, CAMHS etc.
  • Skilled in supporting clients with mental health needs, acute and chronic mental health needs management, complex mental health support including multiple diagnoses, management of challenging behaviour, therapeutic interventions, risk assessment and management including self-harm and suicide, experience of working with mental health act and S117 aftercare;
  • Minimum 2 years post-qualification experience in similar field
  • Up-to-date understanding of the national standards and regulations (e.g. CQC, Care Inspectorate Scotland / Wales);
  • Confident in technology and remote working;
  • Understanding of NMC Codes of practice and Continual Professional Development;
