We are seeking a dedicated Per Diem Licensed Bilingual Clinical Social Worker (LCSW) in Florida to join our team, providing vital behavioral health and psychosocial support to cancer patients. This role involves a range of responsibilities aimed at addressing the emotional and social challenges associated with a cancer diagnosis.

Reporting to the Senior Manager of Clinical Operations, the LCSW/Care Navigator is responsible for conducting initial intake assessments following Medicare and Insurance guidelines and protocols. The LCSW will also be responsible for engaging with Jasper members and their caregivers to plan, coordinate and manage their care. Provide individualized, one-on-one support to oncology patients, encompassing comprehensive navigation assistance throughout their cancer journey. If appropriate, the LCSW/Care Navigator will also provide support with financial toxicity, emotional distress, nutrition, and physical and mental health, providing resources and participating in outreach efforts with the member and/or their caregiver throughout the cancer journey. The LCSW/Care Navigator is expected to be available during  business hours M-F 8-5 CST. 

Role and Responsibilities

  • Mental Health Assessments: Conduct virtual comprehensive mental health evaluations/assessments to identify and address patients' emotional and social issues related to their cancer diagnosis. Utilize tools such as PHQ-9, GAD-7, and SDOH to inform assessments.
  • Therapeutic Treatment: Develop and implement treatment plans focusing on behavior modification, anxiety reduction, and stress management to support patients through their cancer journey.
  • Case Management: Offer ongoing therapeutic support and case management, including cancer navigation services, to assist patients in managing their care effectively.
  • Documentation: Accurately prepare and maintain treatment and progress reports, and ensure documentation meets Private Insurance and Medicare standards. Document in Jasper’s EMR system in accordance with specified guidelines and within a timely manner.
  • Communication & Coordination: Collaborate with other agencies and healthcare professionals to coordinate comprehensive care and support for patients. Identify resources for members to overcome barriers to care, such as financial toxicity, emotional distress, etc. This may involve working with our non-profit partners to get patients the resources they need
  • Crisis Intervention: Provide timely and effective responses to crisis situations, including suicide risk assessments and the development of appropriate intervention plans and referrals.
  • Community Resource Liaison: Guide patients in accessing community resources, acting as an advocate and liaison for patients and their families.

Ongoing Education: Stay informed about local and regional cancer resources to enhance patient support and care.

Qualifications and Education Requirements

  • Must be a Licensed Clinical Social Worker (LCSW) in one or more of the following states: Alabama, Alaska, Arkansas, California, Colorado, Connecticut, Florida, Hawaii, Idaho, Indiana, Louisiana, Michigan, Minnesota, Missouri, Nebraska, Nevada, New Jersey, New Mexico, New York, North Carolina, Oklahoma, Oregon, Rhode Island, South Carolina, Tennessee, Virginia, Washington and Wisconsin. 
  • Oncology experience required
  • Certified Patient Navigator or experienced with patient navigation preferred.

Knowledge, Skills, and Abilities Required

  • Proficient in English and Spanish is a must
  • A readiness to confidently navigate uncertain situations with both patients and colleagues and a readiness to adapt to meet the needs of the patients and the team.
  • Skilled in open, constructive and honest verbal and electronic communication.
  • Excellent interpersonal communication skills.
  • High emotional intelligence & empathy towards patients. 
  • Culturally competent and skilled in working with patients from diverse racial, ethnic, religious and socioeconomic backgrounds.
  • The ability to effectively use technology to deliver high quality care.
  • Ability to multi-task across multiple digital platforms and be adaptable to constant changes and updates to platforms.
  • Knowledge of supportive care principles and techniques for oncology patients.
  • Ability to educate members and/or caregiver’s as to the nature of disease and to provide instruction on proper care and treatment.
  • Ability to adapt to expectations of specific clients and adjust to needs of their member population.
  • Critical thinking skills.
  • Excellent listening, written and communication skills.
  • When communicating with members, providing empathy and understanding.
  • Ability to handle grief.
  • Ability to plan, implement and evaluate individual patient care programs.
  • Ability to gather data, compile information, and prepare reports.
  • Basic computer skills in Microsoft and Google programs.
