We are seeking a dedicated and skilled Remote Registered Nurse to join our virtual healthcare team. In this role, you will be responsible for managing chronic care patients through telehealth consultations, remote monitoring of patient devices, and providing education and support to improve patient outcomes. This position is ideal for a proactive nurse who thrives in a digital environment and is passionate about enhancing the quality of care for patients with chronic conditions

ROLES & RESPONSIBILITIES:

  • Chronic Care Management:
    • Conduct virtual assessments and evaluations of patients with chronic illnesses.
    • Develop and implement individualized care plans in collaboration with interdisciplinary teams.
    • Monitor patient progress and adjust care plans as necessary to achieve health goals.
  • Remote Patient Device Monitoring:
    • Oversee the use of remote monitoring devices (e.g., glucose monitors, blood pressure cuffs, wearable technology).
    • Analyze data from patient devices to identify trends and intervene as needed.
    • Provide timely feedback to patients regarding their device readings and overall health status.
  • Patient Education and Support:
    • Educate patients on disease management, medication adherence, and lifestyle modifications.
    • Offer emotional support and counseling to patients and their families.
    • Facilitate communication between patients and their healthcare providers to ensure continuity of care.
  • Documentation and Compliance:
    • Maintain accurate and up-to-date electronic health records (EHR) for all patient interactions.
    • Ensure compliance with all healthcare regulations and organizational policies.
    • Participate in quality improvement initiatives and contribute to best practice protocols.

QUALIFICATIONS – EDUCATION, WORK EXPERIENCE, CERTIFICATIONS:

  • Current RN licensure
  • Bachelor’s degree in Nursing (BSN) preferred.
  • Minimum of 2 years of nursing experience, preferably in chronic care management or telehealth.
  • Strong understanding of chronic diseases and their management.
  • Proficient in using telehealth platforms and remote monitoring technology.
  • Excellent communication and interpersonal skills, with a patient-centered approach.
  • Ability to work independently and manage time effectively in a remote environment.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Experience with electronic health records (EHR) systems.
  • Familiarity with relevant healthcare regulations (HIPAA, etc.).

TRAVEL REQUIREMENTS & WORKING CONDITIONS:

  • Flexible hours may be required to accommodate patient needs and team schedules.
