Key Responsibilities

  • Coordinate and deliver empathetic care to patients and caregivers.
  • Conduct comprehensive nursing assessments to determine patient and caregiver needs.
  • Develop, implement, and update care plans to guarantee seamless and effective care.
  • Empower patient and caregiver autonomy whenever feasible.
  • Implement established protocols and guidelines in the delivery of hospice care.
  • Facilitate informative patient and caregiver education, allowing ample time for inquiries and discussions.
  • Maintain clear and open communication with interdisciplinary team members, patients, caregivers, and healthcare professionals to ensure continuity of care.
  • Keep supervisors updated on complex patient or caregiver issues while fostering transparent communication.
  • Apply infection control principles and adhere to universal precautions as per accepted professional standards.

Qualifications

  • A Diploma or Associate Degree in Nursing is required; preference is given to candidates with a Bachelor's Degree.
  • A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is essential.
  • Current CPR certification is mandatory; CHPN certification is preferred.
  • At least 1 year of relevant experience for inpatient unit roles and a minimum of 2 years in other nursing positions is required.
  • Direct care experience with patients facing life-limiting illnesses is crucial, including hospice, palliative care, family medicine, internal medicine, geriatrics, oncology, and home healthcare.
