Job Title: Physician/Nurse Practitioner - Health Risk Assessments

Job Overview

Medigence Health is urgently seeking skilled Physicians and Nurse Practitioners to conduct Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace members. This remote position offers the flexibility to perform evaluations in various settings, including home visits, clinics, telemedicine, and long-term care facilities. Key Responsibilities

  • Conduct comprehensive evaluations of medical and social assessments for beneficiary members using Medigence Health’s tablet-based assessment tool.
  • Identify and refer members for routine, urgent, and emergent case management as needed.
  • Provide recommendations for members to follow up with their primary care physicians.
  • Travel within local counties, regions, or states where you hold a current license, particularly in Wisconsin.

Required Skills

  • A minimum of 2 years of direct clinical experience as a Nurse Practitioner or Physician.
  • In-depth knowledge of the HCC model, HEDIS, and STAR quality measures.
  • Current and unrestricted license to practice in Wisconsin, or ability to obtain one.
  • Registered with Medicare and Wisconsin Medicaid.
  • Board certification, preferably in internal medicine, geriatric, or family medicine.
  • Strong communication skills, particularly with elderly and chronically ill members.
  • Proficiency in using computers, tablets, and mobile technology.

Qualifications

  • At least 2 years of hands-on clinical experience.
  • Board certification in a relevant specialty is preferred.
  • A Master’s degree in Nursing or Medicine is required.
