Job Overview

Medigence Health is seeking dedicated and licensed Nurse Practitioners and Physicians in Michigan to deliver comprehensive Health Risk Assessments tailored for Medicare, Medicaid, and Commercial Marketplace members. Our organization operates with a commitment to enhancing patient quality of life through exceptional care and positive experiences.

Key Responsibilities

  • Conduct thorough medical and social assessments utilizing Medigence Health’s tablet-based assessment tool.
  • Identify, refer, and manage the care of members/patients needing routine, urgent, or emergent case management.
  • Recommend follow-up care with primary care physicians as needed.
  • Travel across local counties, regions, and/or states where you maintain an active license.
  • Perform comprehensive assessments via telehealth, ensuring robust audio and video connectivity.
  • Maintain a current unrestricted license in the state of Michigan.
  • Note: Prescribing or altering current treatment plans is not required.

Required Skills

  • A minimum of 2 years of direct experience as a Nurse Practitioner or Physician.
  • Familiarity with HCC model, HEDIS, and STAR quality measures.
  • Current and unrestricted license in the state of Michigan or the ability to obtain one.
  • Registration with both Medicare and Medicaid is necessary.
  • Board certification is required, with a preference for specialties in internal medicine, geriatric medicine, family medicine, or nephrology.
  • Strong communication skills, particularly with elderly and chronically ill patients.
  • Proficiency in telehealth technologies and comfort in utilizing digital tools such as computers, tablets, and mobile devices.

Qualifications

  • Candidates must possess board certification in a relevant specialty, with a preference for internal medicine, geriatric medicine, family medicine, or nephrology.
  • A current, unrestricted Michigan license is mandatory or candidates must be willing to acquire one.
