Position Title: Job Coach, Community Health Worker Training Project

Reports To:   Project Manager, Community Health Worker Training    

Employee Status: Full Time, Exempt                                   

Location/Office: NYC (CAI has a hybrid work schedule of working 2 days in the office and 3 days remote.) 


GENERAL STATEMENT OF RESPONSIBILITIES:

CAI, a national nonprofit organization that provides training, technical assistance, and other support to health care and social services agencies, is seeking a highly motivated professional for the full-time position of Job Coach, Community Health Worker Training and Apprenticeship Project. Under the direction of the Project Manager, Community Health Worker Training, this individual is responsible for managing the delivery of job-readiness Coaching to support trainees in CAI’s Community Health Worker Training Certificate Project (CHWTP)  as well as Coaching to partner agencies to prepare and support them for successful adoption of new trainees and apprentices from the Project. 

JOB SUMMARY: 

We are seeking a highly motivated and organized candidate that has experience in job-readiness, apprenticeship, and workforce development to support the Community Health Worker (CHW) trainees in CAI’s CHWTP. The ideal candidate will have a passion for CAI’s mission, be a team player and a problem-solver, and have a proven track record of successfully supporting job-readiness of new health care and/or non-profit workers. The Job Coach will be responsible for supporting the ongoing management and provision of job readiness and wrap-around support  to assist  CHWTP trainees with successfully completing the program and entering the workforce. The Job Coach will work with trainees to ensure they are receiving the support they need to successfully complete the training certificate program and with agencies that are receiving the trainees to ensure that placement is successful. The Job Coach will work closely with the Project Director, Manager, and partner agencies to support trainee recruitment, ongoing supportive coaching, and placement. This is a highly relationship-focused and detail-oriented position that will be key in the retention of CHWs in the project.

MAJOR TASKS AND RESPONSIBILITIES (include, but are not limited to):

  • Work closely with Project Manager and Program Director to support program recruitment, application, and admissions processes.
  • Build relationships with cadres of trainees as well as the agencies they are placed in.
  • Assist trainees with completing program and placement agency enrollment/onboarding paperwork. 
  • Work closely with supervisors and staff at the placement agencies to make sure the trainees are completing onsite training and skills building and to address the needs of trainees as they emerge.
  • Provide ongoing support, guidance, and job-readiness coaching to CHW trainees through in-person and virtual meetings to ensure they are meeting the requirements of the training certificate and placement project.
  • Identify and address any challenges or obstacles that may be preventing trainees from successfully attending and completing the project.
  • Communicate regularly with trainees and placement agencies to ensure all parties are informed about the progress of the trainees and any issues that need to be addressed.
  • Collaborate with other members of the training and placement team to develop and implement strategies for improving the project and increasing the retention of CHWs.
  • Work closely with the Project Manager and Director to ensure that the support provided to CHWs is tracked and documented, and budgets are maintained.
  • Provide ongoing mentoring and coaching support, including activities such as working together to develop resumes and LinkedIn accounts, setting and monitoring job-seeking goals, and connecting trainees with organizations within the project and partner networks.
  • Provide support for trainees not immediately employed to seek job opportunities and set goals for applying to positions of interest.

MINIMUM EDUCATION, EXPERIENCE AND OTHER SKILLS REQUIRED:

  • BSW, MSW, or a related field or 2-3 years experience as a case manager managing a complex client portfolio 
  • 2 years of experience in community health or workforce development or public health or related field 
  • Previous experience working on job-readiness programs.
  • Strong experience in building relationships and working with diverse groups of people.
  • Excellent communication and interpersonal skills.
  • Strong attention to detail and ability to manage multiple tasks and projects simultaneously.
  • Ability to think creatively and problem-solve.
  • Excellent planning skills, extremely organized, and ability to multi-task.
  • Demonstrated skills in project coordination.
  • Ability to work both independently and as part of an interdisciplinary team. 
  • Flexibility of schedule and ability to travel (approximately 25% local travel-time required).

 PREFERRED:

  • Experience as a Community Health Worker or Health Support Worker or Patient Navigator or similar role, preferred. 
  • Experience working with non-profits or in a mission-driven organization is a plus.
  • Student centered, collaborative, and data driven approach to workforce development.
  • Advanced knowledge of Microsoft Word/Excel/Access/PowerPoint/Outlook.
Compensation
The salary range for this position is $70,000- $75,000. CAI offers a generous benefits package including:
