Office Location:  This position is based out of North Carolina and requires you to live within 75 miles of Trillium’s South Central or Southern Region of our catchment (Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, Wayne, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, and Scotland Counties)

What We’re Looking For

Trillium Health Resources currently has multiple career openings for Transition Coordinator positions throughout our catchment area. Transition Coordinators  are responsible for providing care coordination (treatment planning, assessment, monitoring, educating, and referral/linking) to individuals identified in the Transitions to Community Living Initiative Special Needs Populations (TCLI) by targeting those chronic, unresolved or complex physical, behavioral health, and social determinant needs.  In this role, you'll engage members in the care coordination/care management process with reflective listening and motivational interviewing in order to assess the mental health, medical and social needs of the member to successfully transition to independent living within a timely and effective manner.

On a typical day, you might:

  • Provide care coordination/care management to assigned TCL individuals who may have identified needs with behavioral health, physical health, co-occurring, co-morbid or multi-morbid conditions.
  • Provide education and support to members.
  • Ensure assessments and plans are updated as needed.
  • Collaborate with integrated teams and members.
  • Facilitate comprehensive assessments.

Qualifications: 

Education/Experience:

  1. Master's degree in a human services field or licensure as a RN and one (1) year if relevant experience working directly with individuals with SED or SMI; OR


  1. Bachelor's degree in a human services field or licensure as a RN and three (3) years of relevant experience working directly with individuals with SED or SMI.

 

License/Certification: May be accepted as noted above.

Must have a valid driver’s license
