Job description


As a Health Outreach Coordinator you will be responsible for identifying populations with gaps in care and connecting those patients to care to ensure continuity and timely access to services. This is a centralized position that will provide telephonic care coordination.  You'll also help with patient empanelment and quality improvement practices to monitor patient outcomes and access to care.

  • A stable work schedule. This position will typically work 40 hours per week, Monday - Friday 8 AM - 5 PM. Schedule may flex to include some evening hours as needed.

What You Will Do

  1. Utilizes Epic workbench reports and documentation workflows for each clinic location.
  2. Completes care coordination notes for patients requiring recall.
  3. Verifies and updates the primary care provider/empanelment of patients on the recall lists.
  4. Completes telephonic recall of patients and schedules them in appropriate clinics, which could be related to any service line or department.
  5. Provides warm hand-offs to Nurse Care Managers or Behavioral Health Consultants for additional care management based on risk parameters.
  6. Completes pre-visit planning documentation for those recalled, and capture outside record results in the patient chart for accurate data capture.
  7. Assists with HEDIS chart reviews as needed.
  8. Provides feedback and approaches on Epic clinical documentation optimization.
  9. Assists with other asynchronous office tasks such as clinic-level referrals, prior authorizations, and return phone calls.
  10. Participates in patient retention, customer experience, and related quality improvement activities.


Qualifications You'll Need for This Position

Required: 

  • Completion of high school diploma or equivalent for candidates with MA certification or an accredited nursing program accepted by the Iowa Board of Nursing for candidates with LPN licensure. 
  • A minimum of two years of experience in a clinical role that includes working with UDS measures and closing care gaps.
  • Excellent customer service skills.
  • Effective verbal and written communication skills in English.
  • Excellent interpersonal, teamwork and collaboration skills with ability for a team approach to care plus develop and maintain effective working relationships with a diverse group of individuals.
  • Organization and prioritization skills. 
  • Attention to detail with a high degree of accuracy. 
  • Professional skills with ability to remain composed in high-stress situations and show empathy.
  • Basic computer skills with proficiency using Microsoft Office applications and electronic health record system.
  • Effective problem-solving skills.
  • Licenses & Certifications: Current Medical Assistant Certification or Registration issued by AAMA, AMT, NCCT or NHA or current LPN license recognized by the Iowa Board of Nursing; *Current Basic Life Support (BLS) for Healthcare Providers; *Current Mandatory Reporter Training: Child Abuse and Dependent Adult Abuse recognized by the State of Iowa

* Must be obtained within the Introductory Period if not current.

Preferred: 

  • Effective verbal and written communication skills in Spanish.
  • Completion of Medical Assisting program from an accredited institution for candidates with MA certification.
  • Work experience in a community health center.
  • Experience working in the Epic EHR Software.
