About Cardea Health: 

Cardea Health is an Oakland based nonprofit healthcare organization founded to connect marginalized populations to the clinical and supportive services they need to improve their health, remain stably housed in their community, and age in place. Cardea Health’s programs fill a critical – and growing – gap in the system of care.  Among people experiencing homelessness, a growing number are living with complex medical needs. They face poor quality of life, increased morbidity and mortality, and medical and social destabilization. But they have few housing options: they need a higher level of care than what is offered through permanent supportive housing, but they do not wish to live in an institutional setting and have limited access to hospice or palliative care.  

Cardea Health specifically targets this vulnerable population, providing a range of clinical services within permanent supportive housing sites to both address their medical needs and allow them to live independently in their communities, addressing a critical need within the system of care. 

 

About this role:  

Shift: Full time or Part Time

Hours: 12:30PM - 9:00PM

 

Cardea Health is initiating clinical care and services at the Kelly Cullen Community in the Tenderloin neighborhood of San Francisco. The Kelly Cullen Community is in the Tenderloin neighborhood and is home to 174 individuals with extremely low incomes, including those that experienced homelessness and are currently living with health challenges. On-site social services are run by the Tenderloin Neighborhood Development Corporation, clinical services are provided by Cardea Health.  Medically frail residents are provided nursing care, caregiver support and medical casework up to and including end of life care. 

Cardea Health seeks a highly motivated Registered Nurse with a special interest in working with people who have experienced homelessness to work as a Registered Nurse at the KCC. The Registered Nurse will be the primary point of clinical care for KCC residents. The ideal candidate will be invested in Cardea Health’s mission of serving marginalized populations with compassion and empathy. Cardea clinical staff need to be flexible, patient, and willing to work in challenging environments with a diverse population. Experience with homeless or incarcerated population is highly desirable.  

Qualifications

Required Qualifications: 

  • Hold an associate or bachelor’s degree in nursing. 

  • Current license as Registered Nurse through the California Board of Registered Nursing. 

  • Current Basic Life Support Certification 

  • Current two years of experiences as a Registered Nurse 

Essential duties and responsibilities: 

  • Provide quality care in accordance with Cardea Health policies, procedures, and standards. Ensure that all clients are treated with dignity and respect. 

  • Conduct client intake assessments upon arrival to the program. Identify clinical needs and individualized goals of respite stay. Execute care plans for admitted clients in collaboration with primary and specialty care teams, medical directors, and client. 

  • Provide clients with appropriate information and education to make informed choices related to medical care and treatment that reflects individual preferences. 

  • Maintain communication among other members of the clinical team. Communicate effectively to partner hospitals, community health clinics, homeless service providers, and other collaborating agencies. 

  • Evaluate and triage referrals to the program and communicate with referring partners on referral status. 

  • Work with team members to develop creative solutions to meet the needs of individual clients. 

  • Participate in the collection and maintenance of data on program operations for reporting and analytical purposes. 

  • Ensure adherence to infection control standards on site among staff and residents, including measures designed to stop the spread of Covid-19. 

  • Ensure that clinical documentation is current, timely, and accurate. 

  • Maintain confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA). 

  • Recognize and respond to medical emergencies, follow up with MD communication or 911 activation. 

  • Practice harm reduction principles, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect. 

Physical Demands: 

  • Employees may need to stoop, kneel, crouch, stand, walk, use arms, hands and fingers. Employees will frequently need to communicate and listen. 

  • Employees may need to occasionally lift 100 pounds or more. 

  • Requires clear close vision, distance vision, peripheral vision, depth perception and the ability to adjust focus. 

  • Employees may be occasionally exposed to toxic or caustic chemicals. 

Work Environment:  

  • Employees have direct contact with clients/patients while administering care.  

  • This position will require traveling to other office locations or off-site meetings as required by the job.  

  • Working remotely at the discretion of the Nursing Director.
