Job Overview

We invite you to become a vital part of our esteemed team, dedicated to enhancing the connection between patients and their primary care providers while fostering overall health and wellness. In this pivotal role, you will engage with patients, assisting them in navigating their healthcare journeys, resolving various challenges, and improving communication with their medical practices.

Key Responsibilities

  • Collaborate remotely with medical practice staff and our Case Management team to manage a designated patient population.
  • Operate independently while being an integral member of the team, serving as an extension of the medical practice.
  • Support patients in achieving improved health outcomes by addressing inquiries and concerns, linking them with their medical practices, implementing effective care plans, and closing care gaps.
  • Escalate medical issues appropriately and direct patients to the relevant resources, including specialty care and community organizations.
  • Maintain accurate documentation of patients' medical histories and concerns within the electronic health record (EHR).
  • Familiarize yourself with the referral and prior authorization processes to facilitate patient care.
  • Cultivate and sustain trusting, professional relationships with assigned medical practices and within our organization.
  • Achieve assigned Key Performance Indicators to promote positive patient health outcomes while minimizing healthcare costs.
  • Engage high-risk patients through various effective communication methods, including calls, texts, portals, and emails.
  • Escalate medical concerns to the Health Guide Registered Nurse, Primary Care Physician, or leadership as necessary.
  • Occasionally travel to support primary care physician offices.
  • Adhere to all security policies and procedures to safeguard Protected Health Information (PHI) and company intellectual properties.

Required Skills

  • Bilingual proficiency in Korean and English is required.
  • Certification as a Medical Assistant or as a certified EMT is mandatory.
  • A strong passion for patient coordination and a commitment to serving individuals with complex medical and social needs.
  • Empathy and a readiness to enhance skills in motivational interviewing, critical listening, and behavioral change management to improve patient engagement.
  • Exceptional communication skills and the ability to document patient interactions accurately and efficiently using software.
  • Familiarity with medical terminology and the operational dynamics of primary care practices.
  • Knowledge of social determinants of health.
  • Proficiency in documenting within electronic health records (EHRs) and utilizing care management analytics databases, along with various software programs.
  • Ability to manage multiple priorities while remaining detail-oriented and focused.
  • Familiarity with various electronic health records systems such as EPIC, Athena, and Cerner.
  • Excellent written, visual, and verbal communication skills.
  • Capable of tracking numerous tasks and ensuring timely completion.
  • Proven ability to work independently as part of a patient's overall care team, under the supervision of licensed clinical professionals.
  • Demonstrated success in a remote work environment.

Compensation And Benefits
