Job Overview

We are seeking a dedicated Patient Care Coordinator to join our team as a vital link between patients and their primary care providers. This role is pivotal in enhancing patients’ healthcare experiences and outcomes by facilitating effective communication and support.

Key Responsibilities

  • Collaborate remotely with medical practice staff and our specialized Case Management team to manage a defined group of patients.
  • Function autonomously while also being a crucial collaborative team member, acting as an extension of the medical practice.
  • Assist patients in achieving optimal health outcomes by responding to their inquiries, connecting them with their medical practices, and implementing impactful care plans.
  • Identify care gaps, escalate medical concerns, and guide patients in accessing appropriate resources, including specialty care referrals and community-based organizations (CBOs).
  • Accurately document patients' medical histories and issues within the electronic health record (EHR) system.
  • Navigate the referral and prior authorization procedures effectively.
  • Cultivate and sustain trustworthy, professional relationships with designated medical practices and within our organization.
  • Meet designated Key Performance Indicators (KPIs) to foster positive patient health outcomes and minimize medical expenses.
  • Engage high-risk patients through various communication methods, including calls, texts, portals, and emails.
  • Appropriately escalate medical issues to the Health Guide Registered Nurse, Primary Care Physician, or management.
  • Occasionally travel to supported primary care physician offices as necessary.
  • Adhere to security policies and procedures to safeguard all Protected Health Information (PHI) and company data.

Required Skills

  • Bilingual (Korean/English) - Native or Fluent required.
  • Certified or Registered Medical Assistant or certified EMT required.
  • Strong commitment to patient coordination and serving individuals with intricate medical and social needs.
  • Empathy and proficiency in motivational interviewing, critical listening, and behavioral change management to enhance patient engagement.
  • Excellent communication skills, including the ability to document patient interactions accurately and in real time.
  • Familiarity with medical terminology and the operational structure of primary care practices.
  • Understanding of social determinants of health.
  • Proficient in utilizing electronic health records (EHRs), care management analytics databases, and various software applications.
  • Ability to manage competing priorities while maintaining strong attention to detail.
  • Experience with electronic health records systems such as EPIC, Athena, and Cerner is advantageous.
  • Exceptional verbal, visual, and written communication abilities.
  • Capability to monitor progress on multiple tasks and ensure timely completion.
  • Ability to work independently as part of a patient’s overall care team, under licensed clinical professionals’ supervision.
  • Proven success in a remote work environment.

Compensation And Benefits

The base pay for this position starts at $24+ per hour, with bi-weekly payments. The final compensation will depend on various factors, including skills, experience, education, location, and licensure/certifications.
