Key Responsibilities

  • Conduct regular check-ins with patients to discuss updates, follow-up on appointments, and clarify new medical instructions.
  • Build strong and trusting relationships with patients and their families.
  • Serve as a vital link in the patient care team by maintaining effective communication with healthcare providers.
  • Empower patients to prioritize their health and understand their medical conditions.
  • Advocate for patients by addressing their medical concerns and providing timely responses.
  • Utilize technology to effectively manage and coordinate patient care.
  • Collaborate with the care team and accurately document all patient interactions using Electronic Medical Record (EMR) systems.

Required Skills

  • Display a compassionate approach to nursing care.
  • Possess RN/LPN licensure in any U.S. state located in the Pacific Time Zone.
  • Have a minimum of 4 years of nursing experience, specifically with chronically ill patients.
  • Familiarity with Electronic Medical Records (EMR) systems is essential.
  • Demonstrate proficiency in adapting to new technological tools.
  • Exhibit exceptional communication skills, both in writing and verbally.
  • Effectively manage time and responsibilities while working remotely.

Qualifications

  • A valid RN/LPN licensure is required.
  • At least 4 years of relevant nursing experience, preferably in home health or primary care.
