Job Overview

Key Responsibilities

Conduct regular check-ins with patients to discuss updates, follow-up on appointments, and clarify new medical instructions.
Build strong and trusting relationships with patients and their families.
Serve as a vital link in the patient care team by maintaining effective communication with healthcare providers.
Empower patients to prioritize their health and understand their medical conditions.
Advocate for patients by addressing their medical concerns and providing timely responses.
Utilize technology to effectively manage and coordinate patient care.
Collaborate with the care team and accurately document all patient interactions using Electronic Medical Record (EMR) systems.

Required Skills

Display a compassionate approach to nursing care.
Possess RN/LPN licensure in any U.S. state located in the Pacific Time Zone.
Have a minimum of 4 years of nursing experience, specifically with chronically ill patients.
Familiarity with Electronic Medical Records (EMR) systems is essential.
Demonstrate proficiency in adapting to new technological tools.
Exhibit exceptional communication skills, both in writing and verbally.
Effectively manage time and responsibilities while working remotely.

Qualifications

A valid RN/LPN licensure is required.
