Reporting to the CM Operations Manager, the Care Coordinator will work closely with the Case Managers for reporting, care coordination phone calls, and other administrative duties. The Care Coordinator will increase efficiency of operations by providing general customer support duties and by supporting the Case Manager and operations staff in a team environment.

Candidate must be able to provide and confirm a safe home office environment. Home office must be HIPAA compliant.
