Key responsibilities:

  • Perform daily triage of LTSS Inbox and manage incoming communications.
  • Screen and review 3051 forms for accuracy and duplicative services.
  • Assist with Private Duty Nursing staffing and coordinate with external providers.
  • Maintain documentation and follow up on requests in the LTSS Inbox.

Posting Salary Range: $53,045 - $66,837

Office Location: Remote within Trillium’s catchment area of North Carolina. What We’re Looking For

Trillium Health Resources has a career opening for a LTSS Triage Coordinator to join our Care Management team. The LTSS Triage Coordinator assists members who are eligible for Long Term Support Services Supports and 1915(i) services. This position is responsible for assisting with the coordination of communications and tasks directly related to LTSS services, ensuring members’ needs are met. An ideal candidate would have experience handling large databases, Microsoft applications and working in a fast paced environment. On a typical day, you might:

  • Perform daily triage of LTSS Inbox by reading incoming mail
  • Perform daily triage tasks
  • Screen 3051 forms for accuracy
  • Check for duplicative services
  • Assist with duties and functions related to Private Duty Nursing (PDN) staffing
  • Contact external Providers and agencies as it relates to LTSS
  • Manage follow up information in LTSS Inbox by reviewing mail and sending responses to the requestor
  • Review of all 3051 forms received
  • Maintain documentation

Qualifications: 

Education/Experience:
 Must meet QP status.

 

  1. Fully licensed by the NC governing board regulating Human Services professions; OR

 

Fully licensed RN who is licensed to practice in the state of NC by the NC Board of Nursing who also has four (4) years of full-time experience with the MH/DD/SA population served. OR

 

  1. A graduate of a college or university with a Master’s degree in a human service field and has one year of full-time, pre or post-graduate degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has one year of full-time, pre or postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling; OR

 

  1. A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has two years of full-time, pre or post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; OR

 

  1. A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has four years of full-time, pre or post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.  

 

Additionally, 

If serving members with LTSS needs, the Coordinator must meet the minimum QP requirements defined above (A-D) and additionally shall have at a minimum two (2) years of prior LTSS and/or HCBS coordination, care delivery monitoring and care management experience. This experience may be concurrent with the years of experience required to become a Qualified Professional. 

 

Preferred Experience: 
 Experience with understanding triage and ability to prioritize urgent tasks

License/Certification: 
 Must have a valid driver’s license
