As the Manager of Chronic Care Management (CCM) APPRs, you will be responsible for managing APPR clinical reviewers ensuring effective team performance by setting clear goals, providing regular feedback, and foster a collaborative and inclusive work environment. The Manager of CCM APPRs collaborates with the Sr. Manager of CCM to address staffing needs, manage employee relations, and handle escalation issues effectively. Additionally, responsible for chart reviews, onboarding process, support APPRs with clinical education, auditing, and RPMI assignments.
REPORTS TO: Sr Manager of Clinical Reviewers
PRIMARY RESPONSIBILITIES:
Manage the APPR employees ensuring productivity through regular feedback while providing day-to-day operational oversight
Interview APPR applicants, both employee and contractors
Assist with on-boarding newly hired employees and contractors. Conduct ongoing training for systems or process updates.
Monitor productivity for FT team and assist with 1099 if needed
Responsible for approval of PTO requests, CME/license reimbursement approvals for FT APPR
Assist with RPMI assignments
Serve as a resource to provide ongoing clinical education specific to chronic care management/remote patient monitoring/behavioral health
Monitor and assure that the staff have the resources to get their work done in an efficient manner.
Participate in committees that directly enhance the work of chronic care management.
Conduct occasional chart reviews to ensure that current processes are followed.
Be a liaison working with various departments: IT, Credentialing, Human Resources to assure that staff is appropriately trained, and credentialed
Monitor various chats/conversations and answer questions
Complete approximately 25% of expected productivity for chart reviews (may be adjusted based on administrative tasks schedule)

Minimum Requirements
Must have an active and unrestricted license in TX, CA or NJ. MSN with National Board Certification (AANP or ANCC)
FNP/AGNP
At least 2 years of APP experience preferably in primary care, SNF/LTAC, chronic care management. Remote work experience is beneficial.
PCC, Teams, Microsoft office
Detail oriented, fast-paced environment, exceptional communication skills, ability to work independently and in a team
