The Team Lead, DSNP Program - Care Coordination Team provides direct supervision, coaching, and support to non-clinical Care Coordinators and Community Health Workers. This member-facing team plays a vital role in onboarding new members and establishing early trust in the Care Team. Responsibilities include engaging new members, completing HRAs and other assessments promptly, and assisting members in understanding their plan and benefits. The team supports various administrative duties within the Care Team, including handling inbound and outbound calls, ensuring service and quality levels are met, and providing active support to care managers with member outreach. This role focuses on promoting member health and wellness, supporting member-identified care plan outcomes, and ensuring excellence in service, compliance, and documentation. 

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES 

 

  • Orient new members to the Belong Health DSNP program. 
  • Monitor staff performance and provide ongoing coaching and support in achieving key service and compliance metrics.  
  • Ensure the team prioritizes and improves member engagement while delivering high quality customer interactions.  
  • Educate and coach staff on plan benefits, in-home visits, motivational interviewing, community resource linkages, and follow-up care. 
  • Oversee member care plan implementation in accordance with Interdisciplinary Care Team recommendations. 
  • Assign incoming calls, monitor phone queues, and handle escalated issues. 
  • Collaborate with internal teams to resolve member concerns and provide outstanding service. 
  • Facilitate provider meetings as needed to support member health needs. 
  • Review time and productivity weekly and submit time for compensation of non-exempt employees. 
  • Uphold a culture of accountability, empathy, innovation, and member-first service. 
  • Other duties as assigned.
