Key responsibilities:

Manage a caseload of high needs clients and develop individualized service plans.

Conduct assessments and provide ongoing support and advocacy for clients.

Coordinate with community resources and service providers to ensure client needs are met.

Document client progress and maintain accurate case records.

EMPACT – Suicide Prevention Center offers comprehensive crisis and community behavioral health services to children, adults, and families. We provide services in Maricopa and Pinal counties with offices located in Tempe, Phoenix, Glendale, Mesa, Apache Junction, San Tan Valley, and the City of Maricopa. EMPACT-SPC is committed to working collaboratively with our public and private partners to solve community problems. We have the resources and expertise to address issues related to behavioral health, affordable housing, children and youth, employment, crisis intervention/suicide prevention, military veterans, and community and cultural education.
