FLSA Status: Exempt

Posting Salary Range: $70,019 - $79,568

Office Location: Remote within Trillium’s North Central or Southern Region of our catchment area, which includes North Carolina Counties: Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrell, Washington, Wilson, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, and Scotland.

What We’re Looking For

Trillium Health Resources has a career opening for a TCL District Supervisor to join our Transitions to Community (TCL) Team! The TCL District Supervisor oversees a skilled, multi-disciplinary team of professionals who provide localized support within a managed care environment to members who receive Transition to Community and Behavioral Health services. As the District Supervisor, you will provide oversight to ensure members receive whole person care focused on all members’ needs including physical health, behavioral health, and social determinants of health/unmet health resource needs that helps to establish positive outcomes for members and transform their lives for the better. On a typical day, you might:

  • Provide direct supervision and clinical oversight for assigned teams of localized, multi-disciplinary Transition team supporting members with behavioral health conditions.
     
  • Ensure staff utilize workflows 
     
  • Establish a team based, person-centered approach to coordination of care to be implemented by all team members that effectively manages a member’s physical health, behavioral health, and social determinant of health needs using established workflows and processes effectively and efficiently.
     
  • Use knowledge and skills to create and identify methodologies and tools to be used in the execution of care management and coordination.
     
  • Coach team members to use on member engagement practices that support the engagement of members throughout the transition process and continuum of care.

Qualifications: 

Education/Experience:
 
Must have a Master’s degree or be fully licensed as an RN and have three (3) years of experience providing care management, case management, or care coordination to the MH/SU/BH population.
 

License/Certification: 
 
North Carolina fully licensed LCSW, LCMHC, LPA, LMFT, LCAS or RN required.
 Must have a valid driver’s license

Location:
 
Remote within Trillium’s North Central or Southern Region of our catchment area, which includes North Carolina Counties: Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrell, Washington, Wilson, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, and Scotland. 
 Must reside in NC to be considered for remote status. 
 Must be able to travel to a Trillium office location and within catchment as required.
