Job description

The ideal candidate will have experience working with individuals facing homelessness, substance use issues, or serious mental illness (SMI). Key qualifications include familiarity with fair housing laws, community programs, and HUD-funded initiatives. HMIS (Homeless Management Information System) experience is a plus for this role. The position involves substantial travel, up to 75%, for community outreach and events in various locations, including Elko, Quad, and/or Nye County, Nevada.

Position Purpose: The Housing Management Specialist plays a vital role in supporting individuals and families experiencing housing instability by conducting needs assessments, facilitating access to housing resources, and promoting long-term housing retention. This position is remote and works collaboratively with community partners, housing providers, internal care teams, and closed-loop referral systems to coordinate services, support housing placements, and contribute to the success and sustainability of housing initiatives. Additionally, this role will play an active part in supporting the implementation of Nevada Medicaid’s upcoming In Lieu of Services (ILOS) for Housing Supports & Services.

Position Location:

SilverSummit Healthplan is expanding its housing support services across Rural Nevada and is currently hiring three Housing Management Specialist positions to serve designated regions.

We are seeking qualified, community-based candidates in the following areas:

  • Housing Management Specialist- Located in Elko-Elko County
  • Housing Management Specialist- Located in Pahrump- Nye County
  • Housing Management Specialist- Located in Douglas, Carson, Lyon, Storey, or Churchill County. 

These roles will support local efforts to address housing instability, coordinate services, and connect individuals to vital housing, case management, and behavioral health resources.

Key Responsibilities:

  • Conduct housing needs assessments and screenings for eligibility into various housing programs.
  • Support members in completing housing applications and required documentation.
  • Coordinate referrals for shelter, transitional housing, sober living, and permanent housing programs.
  • Assist in tracking member progress and maintaining accurate, timely documentation within EHR systems.
  • Maintain knowledge of local housing resources, Continuums of Care, and program eligibility criteria.
  • Collaborate with housing case managers, community organizations, and providers to support housing placements.
  • Provide follow-up and care coordination to support housing retention and overall member stability.
  • Attend community events, meetings and trainings related to housing programs and community services.
  • Ensure compliance with all relevant regulations and organizational procedures.

Education/Experience:

Minimum Qualifications:

  • Associate degree or equivalent experience including lived experience (Bachelor’s preferred in healthcare, social work, or related field).
  • 1-2 years of experience in human services, case management, housing, or a behavioral health-related setting.
  • Knowledge of local housing systems and services, including shelter, transitional, and supportive housing.
  • Understanding of trauma-informed care and evidence-based housing practices (e.g., Housing First).
  • Proficiency in Microsoft Office and ability to document in electronic health records (EHR).

Preferred Qualifications:

  • Experience working with individuals experiencing homelessness, substance use, or serious mental illness (SMI).
  • Familiarity with fair housing laws, community programs, and HUD-funded programs.
  • HMIS (Homeless Management Information System) & Housing Assessment experience preferred.

License/Certification: Valid state driver’s license required.

Pay Range: $57,700 - $80,800 per year.
