Job description

Pay Range: $81,500 - $86,500/year

Location: Remote, based in North LA County

Travel: 50 to 75% time spent in the field. Territory will span from North LA County/Ventury County up through Kern County/Fresno County

Schedule: Monday through Friday - business hours. Some weekends and evenings may be required.

JOB SUMMARY

The MSOS (Masonic Senior Outreach Service) Care Manager is responsible for the coordination and management of care services for senior members of the fraternity, needing assistance with problems associated with aging and finances.

About Us:

Masonic Homes operates two retirement homes, one in Union City and another in Covina. Additionally, it offers outreach and youth programs that support hundreds of families, both with and without Masonic affiliation.

Our campus is home to about 80 residents who live in comfortable one-bedroom apartments designed for independent and assisted living. Residents choose the Masonic Home at Covina for its intimate setting, vibrant quality of life, and, perhaps most of all, the sense of community created by fellow residents and staff.

ESSENTIAL FUNCTIONS

  • Performs assessment to evaluate physical, functional, financial, emotional, and environmental status of clients.
  • Develops, monitors, and maintains individual care plans for clients to provide appropriate and adequate resources to support their health, safety, and changing needs.
  • Understands basic concepts of confidential of client information and requirements for permissible disclosure.
  • Develops and maintains personal connections with clients as well as client families, peers, Masonic Homes of California departments, Masonic Lodges, vendors and external agencies involved in clients care plans.
  • Maintains a regular visit schedule with clients.
  • Informs clients and appropriate family members of all public benefits to which they may be entitled and provides guidance in obtaining these benefits.
  • Maintains ongoing open communication among all care providers, lodges, and family within privacy/confidentiality requirements.
  • When necessary, acts as an advocate for client to ensure fair and equitable outcomes.
  • Possesses well-versed knowledge about all Masonic Homes of California programs and services.
  • Develops, monitors, and adheres to client budget and requests for assistance for extraordinary occurrences based on current information and according to MOS (Masonic Outreach Services) guidelines. Responsible for reporting any major changes in status to management in a timely manner.
  • Receives and reviews reports, and correspondence as needed.
  • Performs timely updating of and maintains accurate information on the online database, demonstrating a thorough knowledge of the system.
  • Performs related duties as assigned.
  • Travel throughout assigned regions and to MHs campuses. Occasional overnight and out-of-state travel is also required. No on-call duties required.
  • Participate in lodge presentations regarding program services, on occasion.

Requirements

SKILLS, ABILITIES AND EXPERIENCE

  • BA is required +three or more years of work experience in related field.
    • Master's Degree preferred + two or more years of work experience in related field.
  • Knowledge of the aging process, health, and social service system
  • Experience with obtaining benefits through Medicare, Medi-Cal, and other insurance.
  • Must be able to exercise independent judgment, demonstrate good assessment skills and relate effectively to the elderly, professional associates, and clients’ families.

ADDITIONAL REQUIREMENTS/LICENSES/CERTIFICATES

  • Must possess and maintain a valid California driver’s license without restrictions.
