Western Health at Home is seeking a dynamic Nurse practitioner who is passionate about delivering timely, evidence-based, and patient- centred care in the community.
This is a unique opportunity to work autonomously and collaboratively, providing acute-level care in the home across multiple community-based services, including Residential In-Reach, GEM at Home, and other at home programs.
About the Role
As a Nurse Practitioner in the Western Health at Home division, you will:
- Deliver advanced, person-centred care to adults aged 65+ in their homes or residential aged care facilities.
- Provide comprehensive clinical assessments, diagnosis, and management of complex conditions in the community.
- Act as a key clinical leader across services, supporting seamless integration between hospital and home-based care.
- Work autonomously and collaboratively with multidisciplinary teams to enhance care pathways and continuity.
- Contribute to digital health transformation, quality improvement, and service redesign to meet evolving community needs.
What you’ll bring:
- Endorsement as a Nurse Practitioner.
- Proven advanced clinical skills in community, aged, or subacute care.
