šÆ What Youāll Do in This Role:
Home Care, RN Case Manager, where you'll play a vital role in assessing patient needs, creating personalized care plans, and coordinating with a multidisciplinary team to deliver high-quality skilled nursing rights in the comfort of patientsā homes. This position calls for a clinically strong, compassionate nurse with excellent communication and organizational skills. Compensation is offered on a paid-per-visit basis.

ā What Youāll Need:
Qualifications:
⢠š§āāļø Active Registered Nurse (RN) license in the state of Michigan
⢠š Degree in Nursing; bachelorās degree in nursing (BSN) preferred
⢠š
Minimum of 1 year of nursing experience
⢠š” Previous experience in home health, case management, or a related field preferred
