The primary purpose of the job is to:

As part of the Care Management/Case Management department, the Community Health Navigator supports the development and execution of Blue Cross Complete of Michigan’s member outreach strategy to impact health outcomes, health care costs, and member engagement. This position establishes and cultivates relationships with members, community stakeholders, and providers via phone, mail, e-mail, and personal visits. Face-to-face member visits at their home or a community organization are essential to the Community Health Navigator role.

Job-Related Qualifications/Knowledge/Skills/Abilities:

  • Required: Associate’s Degree in Healthcare, Human Services, related field, or equivalent work experience. Bachelor’s Degree preferred.
  • Required: Must reside in Oakland, Macomb, or Wayne County.
  • Required: Minimum of two (2) years of health care in an office/clinic/hospital setting or human services experience or equivalent school experience.
  • Required: Must have a valid Michigan Driver’s License, valid car insurance, and reliable transportation.
  • Required: Traveling to and accessing members' homes and community locations within assigned territory.
  • Required: Basic knowledge and understanding of managed care, medical terminology, and disease processes.
  • Required: Must be able to lift up to 25 pounds.
  • Preferred: Community-based experience through volunteerism, employment, or education.
  • Preferred: Effectively communicating preventive health and health management philosophies to individual members, small groups, and external agencies.
  • Preferred: Comprehensive knowledge of Medicaid requirements, plans, policies, and procedures.
  • Preferred: Bilingual Spanish or Arabic
