Role:
The Social Work Preceptor provides clinical consultation, intervention, and carries out educational opportunities for Social Workers (SWs). Orients the Social Worker/Intern to their role within the Company and ensures s/he has successfully acquired knowledge and skills to be successful in the position.

Qualifications:

  • Master’s degree or above in Social Work (CSWE accredited)
  • Florida LCSW preferred
  • Minimum of three (3) years’ experience including social work/counseling internship in health care/hospital setting with a minimum tenure at a Chapters’ affiliate of one (1) year
  • Experience related to the concepts and practices of death and dying with children and adults
  • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system   
  • Critical thinking and problem solving skills
  • Ability to effectively instruct others (staff and clients)
  • Excellent written and oral communication skills; ability to communicate effectively in English
  • Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload
  • Able to remain calm, professional and trouble shoot patient/family crisis situations
  • Able to demonstrate high ethical standards in areas of Hospice mission, confidentiality, sensitivity, and flexibility
  • Mobile Driver - Valid driver's license and automobile insurance per Company policy
  • Ability to travel to off-site locations
  • Ability to physically access any home, hospital, nursing home, and ALF (i.e., navigate stairs, narrow spaces, environments with lack of air conditioning, etc.)
  • Endurance to manage demands of a hospice field work position (i.e., time, travel, home/facility arrangements, stress)
  • Must remain in good standing from a performance perspective in delivering training to those who provide social services to patients

Job Responsibilities:

  • Assists in orienting new employees, students, volunteers, students, and interns.  Introduces new Social Workers and interns to team and work environment.
  • Collaborates/coordinates with Director of Support Services and Regional Directors regarding social services and bereavement programs, policies, and standards to maintain consistency of practice, establishes relationships, and broadens the scope of the programs.
  • Assists other clinical staff in understanding family dynamics and developing a plan to address issues in a nonjudgmental manner.
  • Guides all new Social Workers in all aspects of their roles and performance expectations including framework for evaluating aspects of care, resolving problem/high risk issues, discharge planning, EMR and paper documentation and maintaining the Plan of Care.  Provides prompt and consistent feedback, identifying areas in need of further development and plans for opportunities for improvement to the SW staff and the Regional Director.
  • Demonstrates safe patient care during learning experience and adherence to Company policies.
  • Completes Preceptor Training program and associated update classes successfully.
  • Promotes development of critical thinking and clinical judgement using appropriate adult learning styles.
  • Validates competency of Social Workers and clinicians in performance and documentation of care.
  • Submits timely documentation as needed, during preceptor experience to Clinical Manager, Regional Director and Training and Development department.
  • Performs other duties as assigned.
