About this Role
Belle is hiring remote Physician Assistants, Clinical Nurse Specialists or Nurse Practitioners that are licensed in the state of Florida. The primary purpose of the role is to conduct telehealth Annual Wellness Assessments and establish Chronic Care Management (CCM) care plans for qualifying, polychronic members. This role is 100% remote, allowing you to utilize your healthcare expertise while maintaining a flexible working environment. In this position, you will perform wellness evaluations, gather comprehensive patient histories, and collaborate with our team to enhance patients’ overall health. Anticipated availability is 9am-6pm EST, Monday-Friday.
