Case Managers provide services to participants living in the community with persistent mental health challenges, in addition to individuals with co-occurring conditions. After assessing the participants' needs the Targeted Case Manager helps the participant with obtaining resources and services in the community, in a partnership that focuses on recovery and the achievement of the participant's goals for community living.
The Case Manager is a full time position, Monday - Friday, 8:30 AM - 5:00 PM (with the opportunity to create a flexible schedule with manager's approval). This is a hybrid work from home position, with required in-person participant visits and office days and occasional work from home options.
