Compensation Range: $24-28 per hour

Compensation will be dependent upon geographic region, education, and experience

Job Description

Coordinates patient care between internal and external providers to improve patient experience and successful transition to the next level of care. Meets with patients on a regular basis to assess aftercare needs and works closely with treatment team and clinical outreach to identify appropriate referrals and secure aftercare appointments.

Responsibilities:

  • Demonstrates the ability to develop a thorough continuing care plan that is in alignment with treatment team recommendations, business development and patient’s individual needs
  • Timely communication and documentation in compliance with HIPPA requirements
  • Establish positive working relationships with clinicians, medical providers, EAPs, and Veteran Association Accounts  
  • Identify and overcome potential barriers to patient care
  • Provides case management necessary to patient success in current treatment program and next steps in recovery process. 
  • Addresses discharge planning needs and concerns from patients.  Activates Discharge Plan within 24 hours, completes and reviews with the patient prior to discharge.
  • Responsible for the Coordination of Care within 72 hours
  • Meets weekly or as needed with patient
  • Attends weekly Treatment Team meetings to gain needed information from therapists and to communicate important information about each patient’s continuing care plan
  • Completes Discharge Planning Progress Notes for each patient
  • Documents outbound referrals in the client’s chart and salesforce
  • Maintain knowledge of client discharge date through assigned therapist and admissions team (as dates may change in accordance with insurance)
  • Lead aftercare group to educate patients on levels of care and importance to their recovery
  • Must meet with patient a minimum of 3 sessions
  • Maintain timely scheduled initial sessions with patients within 6 days of intake.
  • Must meet with patient 1 week before discharge to confirm appointments are secured for client with outside providers and review discharge plan with patient. (IOP, Psychiatrist and Therapist appointments for client before discharge, securing bed in Sober Living, etc.)

This is a full-time position that will work Monday-Friday 8:30am to 5:00pm.

Qualifications
  • Valid CCADC certification and/or 2 years experience in a mental health and/or chemical dependency setting required
  • Experience working with clients and documentation in a hospital and/or mental health facility required
  • At least 3 years' experience in care coordination ​or customer service.  ​Experience in clinical aspects of behavioral medicine preferred.
  • Good verbal and written communication skills.
  • Computer skills and proficiency in Microsoft Office.
  • Strong analytical thinking and the ability to handle multiple tasks concurrently.
  • Excellent customer service.
  • Compassion and empathy.
  • Ability to travel.
